*Please note: We DO NOT bulk bill under Medicare or bill Third Parties (Work-Cover/CTP insurance)
We require full payment of fee's on the day of your consultation. Please see below for more details.

We Do Not Bulk Bill under Medicare.
We accept Cash, Eftpos, MasterCard, Visa & Cheque. We do not accept Diners or American Express Cards.
EFTPOS & HICAPS Facilities are available for your convenience and receipts will be issued for both.
PRIVATE HEALTH INSURANCE
If you have private health insurance please bring your card along and we can process your claim on the spot and you will only need to pay the gap payment. For details on your health fund rebate, please contact your health fund.
The Item Number for an Assessment Consultation is #500 & Item Number #505 for a Subsequent Consultation.
If you have been referred to our practice by your GP under the Medicare Chronic Disease Management Plan (CDM) - previously known as the Enhanced Primary Care (EPC) Please inform our Reception Staff upon arrival, so that the necessary paperwork can be processed.
Please be advised that our clinic Does Not Bulk Bill under Medicare.
As we are a private physiotherapy clinic, you are required to pay our full fee at the time of your consultation and then claim the Medicare rebate of $53.80 back from Medicare.
Here at The Headache, Neck & Jaw Clinic we have the facilities to process your Medicare Rebate through our Hicaps Machine. Please note: that we will need your Medicare card and a Bank Card (either a Savings or Cheque Account) to process your rebate. Medicare does not offer a rebate to credit cards. If you do not have either card, we ask that you visit a Medicare Office and claim your rebate with them.

The Headache, Neck & Jaw Clinic does not bill third parties. We request that patients pay on the day of their treatment and claim directly through Work Cover. *Please note: Work Cover payments do not adequately cover the costs for treatment, please contact your Work Cover Case Manager for more information. You must provide a Work Cover Medical Certificate to commence your treatment.
COMPULSORY THIRD PARTY CLAIMS (CTP)
The Headache, Neck & Jaw Clinic does not bill third parties. We request that patients pay on the day of their treatment and claim directly through their CTP Insurer. *Please note: CTP payments do not adequately cover the costs for treatment, please contact your Claims Manager for more information. You must provide a Medical Certificate to commence your treatment.
DEPARTMENT OF VETERANS AFFAIRS (DVA)
If you wish to be treated under the DVA Scheme you can book appointments between 10.00am and 3.00pm with either Jessica Dowling, Emily Fox, Matthew Altoft, Reegan Emslie or Callum White, at either one of our Clinics.
Treatment by Scott Cook, Nigel Smith, Karri Field & Trina Morris will require a full consultation fee to be paid by the patient. If you have been referred to our practice by your GP under the DVA scheme you must provide a D904 referral from your Doctor and show your DVA Card in order for the necessary paperwork to be completed.
As a courtesy we send an SMS reminder the day before your appointment. We ask that patients take responsibility for remembering appointments. DO NOT REPLY TO OUR SMS, WE DO NOT RECEIVE REPLIES. It is far quicker and more efficient for you to ring if you wish to change your appointment.
